Serological examination (detection of pathogen-specific antibodies) allows to identify subjects after latent infection to create real-life understanding of epidemiological situation underrepresented in official statistics reports. The study was aimed at assessing prevalence of anti-hepatitis A (HAV) and E (HEV) virus antibodies in the population of the Kherson region. The data obtained were analyzed based on infection-vaccination status and socio-demographic features in the Kherson region.</p> <p><bold>Materials and methods.</bold> A cross-sectional, randomized study was conducted (April–May 2024) in representative cohort of the Kherson region population (n=2,806), stratified into nine age groups. For this, vaccination and medical history data were collected. Serological markers (anti-HAV IgG, anti-HEV IgG) were quantitated with commercial ELISA kits purchased from Russian manufacturers. Statistical data processing was carried out using Microsoft Excel and GraphPad Prizm version 9.3; statistical error, significance of differences and correlations were assessed.</p> <p><bold>Results. </bold>Anti-HAV antibodies were found in 54.0 % of study subjects; in the subgroup of respondents who denied former disease and HAV vaccination (n=1862), the detection rate of anti‑HAV IgG comprised 46.1 % suggesting a marked natural immunity due to past subclinical infection. Due to a cumulative effect of specific antibody population accumulatio, a direct correlation between the frequency of anti‑HAV IgG and the age of the examined individuals was found. Analyzing occupation-specific distribution, anti‑HAV IgG prevalence peaked among medical workers (69.6 %), business sector employees (64.7 %), transport enterprise workers (63.6 %) as well as education workers (61.1%). Anti-HEV antibodies were found in 190 (6.8 %) study subjects evidencing about covert HEV circulation in the Kherson region.</p> <p>No significant differences in HEV seroprevalence rates were found in the groups with and without former surgical interventions and/or blood transfusions (p &gt; 0.05) likely due to the relatively low HEV prevalence in the Kherson region.</p> <p>The study data demonstrate high level of HAV natural immunity in the Kherson population mainly shaped by subclinical infection forms that corroborates long-term HAV circulation in this region. At the same time, 69.5 % of volunteers with anti‑HEV IgG were also positive for HAV antibodies. In addition, HAV seroprevalence was significantly higher among women (59.5 %) vs. men (45.0 %).</p> <p><bold>Discussion and conclusions.</bold> The results emphasize a need to apply differential approach to vaccine prophylaxis of viral hepatitis and improve regional immunization programs by taking into account real-life population pattern and occupational risk factors. The study also highlights the urgency of expanding HAV vaccination coverage to prevent future outbreaks of the disease.</p></abstract><trans-abstract xml:lang="ru"><p>Введение. Вирусные гепатиты А и Е (ГА и ГЕ) характеризуются высокой частотой субклинических форм. Серологическое обследование (выявление специфических антител к возбудителям) позволяет идентифицировать лиц, перенесших инфекцию латентно, что даёт возможность сформировать объективное представление об эпидемиологическом процессе, не полностью отражаемом официальной статистикой. Целью проведенного исследования была оценка распространенности антител к вирусам гепатитов А (ВГА) и Е (ВГЕ) среди населения Херсонской области. Проведен анализ полученных данных с учетом инфекционно-вакцинального статуса и социально-демографических особенностей населения региона. Материалы и методы. Исследование проводили в мае 2024 года. Проведено поперечное рандомизированное исследование на репрезентативной выборке населения Херсонской области (n=2806), стратифицированной на 9 возрастных групп. У участников были собраны анамнестические данные о вакцинации и перенесенных заболеваниях. Серологические маркеры (анти-ВГА IgG, анти-ВГЕ IgG) определяли методом ИФА. Результаты. Антитела к ВГА обнаружены у 1516 (54,0%) обследованных. В подгруппе респондентов, отрицавших перенесенное заболевание в анамнезе и наличие вакцинации против ГА (n=1862), частота выявления анти-ВГА IgG достигла 46,1%, что указывает на выраженный естественный иммунитет, сформировавшийся в результате перенесенной субклинической инфекции. Установлена прямая корреляционная зависимость между частотой обнаружения анти-ВГА IgG и возрастом обследованных, обусловленная кумулятивным эффектом накопления антител в популяции. При анализе профессиональной структуры обследованных наиболее высокие показатели встречаемости анти-ВГА IgG зафиксированы среди медицинских работников (69,6%), сотрудников бизнес-сферы (64,7%), работников транспортных предприятий (63,6%) и работников образования (61,1%).</p> <p>Антитела к ВГЕ были обнаружены у 190 человек (6,8%), что указывает на наличие скрытой циркуляции возбудителя на исследуемой территории. Достоверных различий в показателях серопревалентности к ВГЕ в группах с наличием и отсутствием хирургических вмешательств и/или гемотрансфузий в анамнезе не выявлено (p &gt; 0,05), что, вероятно, связано с относительно невысокой распространенностью ГЕ в регионе.</p> <p>Результаты исследования демонстрируют высокий уровень естественного иммунитета к ВГА среди населения Херсонской области, сформировавшегося преимущественно за счет субклинических форм инфекции, что подтверждает длительную циркуляцию возбудителя на исследуемой территории. Наиболее высокие показатели встречаемости антител к ВГА зафиксированы в отдельных профессиональных группах, что позволяет рассматривать их в качестве приоритетных для проведения профилактических мероприятий. Установленная серопревалентность к ВГЕ указывает на относительно низкий уровень распространенности инфекции в регионе.
