Neuroinflammation is one of the key factors in neurodegeneration pathogenesis. Chronic infections and parasitic infestations contribute significantly to neuroinflammation. It has been evident about a link between systemic inflammation, disorders of cellular and humoral immunity and MCI progression. In this context, of particular interest is a role for a common intracellular pathogen <italic>Toxoplasma gondii</italic> may be involved in MCI pathogenesis and clinical dynamics. Currently, the frequency and clinical significance of toxoplasmosis reactivation in MCI remain unknown. Given <italic>Toxoplasma gondii</italic>-related immunotropic effects, it is also relevant to investigate an association between latent toxoplasmosis reactivation, immune parameters and inflammatory mediators in MCI. The study was aimed at assessing the frequency of chronic toxoplasmosis reactivation and its associations with cell immunity, levels of main cytokines and chemokines along with clinical dynamics in MCI. For this, individuals were divided into the following groups: main group included 130 patients with MCI, and comparison group — 81 patients with subjective cognitive impairment (preMCI). The serum was tested for anti-<italic>Toxoplasma gondii</italic> IgG, IgM, and IgA antibodies using enzyme-linked immunosorbent assay, the concentration of cytokines and chemokines was determined by multiplex assay. Cell immunity was assessed using flow cytometry. The Kruskal–Wallis test was used to assess the significance of differences, followed by pairwise comparisons with Mann–Whitney test. An increased frequency of latent toxoplasmosis reactivation (positive for anti-<italic>Toxoplasma gondii</italic> IgA in combination with IgG &gt; 50 IU/mL) was observed in MCI. In MCI patients, reactivation of toxoplasmosis was associated with impaired activation of Th1-cell immunity (lower total and subset T-cell counts including CD4<sup>+</sup>CD57<sup>+</sup> T-cells and CD8<sup>+</sup>CD57<sup>+</sup> T-cells), as well as increased levels and activation of NK-cells. Higher anti-<italic>Toxoplasma gondii</italic> IgG level in patients was associated with elevated neuroinflammatory markers CCL4 and CCL20, and with less favorable one-year-follow-up clinical dynamics. The results suggest that reactivation of latent toxoplasmosis in MCI may be clinically relevant, and it is important to determine not only anti-<italic>Toxoplasma gondii</italic> IgG and IgM, but also IgA to detect it.</p></abstract><trans-abstract xml:lang="ru"><p>Умеренные когнитивные расстройства (MCI) в ряде случаев предшествуют развитию деменции. Нейровоспаление — один из ключевых факторов патогенеза нейродегенерации. Хронические инфекции и паразитарные инвазии вносят важный вклад в развитие и поддержание нейровоспаления. Имеются данные о связи риска прогрессирования MCI с системным воспалением, расстройствами клеточного и гуморального иммунитета. В этом контексте привлекает внимание возможная роль <italic>Toxoplasma gondii</italic> — распространенного внутриклеточного патогена — в патогенезе и прогрессировании MCI. В настоящее время частота и клиническая роль реактивации хронического токсоплазмоза при MCI остается неизвестной. С учетом иммунотропного действия <italic>Toxoplasma gondii</italic> актуально также исследование связи реактивации латентного токсоплазмоза с параметрами иммунитета и содержанием медиаторов воспаления при MCI. Цель работы — изучение частоты реактивации хронического токсоплазмоза и ее ассоциаций с параметрами клеточного иммунитета, уровнем основных цитокинов и хемокинов, а также динамикой когнитивных нарушений при MCI. В основную группу вошли 130 пациентов с MCI, в группу сравнения — 81 человек с легкими когнитивными расстройствами (preMCI). В сыворотке крови определяли антитела IgG, IgM и IgA к <italic>Toxoplasma gondii</italic> методом иммуноферментного анализа, концентрацию цитокинов и хемокинов — методом мультиплексного анализа. Параметры клеточного иммунитета определяли с помощью проточной цитометрии. Для оценки достоверности различий использовали критерий Краскелла–Уоллеса с последующими попарными сравнениями по критерию Манна–Уитни. Впервые показана повышенная частота реактивации латентного токсоплазмоза (позитивный результат определения IgA к <italic>Toxoplasma gondii</italic> в сочетании с содержанием IgG к <italic>Toxoplasma gondii</italic> более 50 МЕ/мл) при MCI. У пациентов с MCI при реактивации латентного токсоплазмоза выявлены признаки нарушений активации Th1-звена клеточного иммунитета (более низкое содержание T-клеток и их субпопуляций: CD57<sup>+</sup> терминально дифференцированных T-хелперов и CD57<sup>+</sup> терминально дифференцированных цитотоксических T-клеток), повышение содержания и активации NK-клеток. Показано, что высокое содержание IgG к <italic>Toxoplasma gondii</italic> у пациентов связано с увеличением уровня нейровоспалительных маркеров CCL4 и CCL20, а также с менее благоприятной клинической динамикой в течение 1 года.
