In a number of patients, despite achieving control and undetectable viral load, the expected recovery of immunologic surveillance and adequate interaction of innate factors are not achieved. The aim of the present study was to comparatively evaluate the state of innate immunity factors in patients with HIV infection. <italic>Materials and methods.</italic> 2 main groups were formed consisting of 56 subjects with HIV infection — with undetectable viral load and with detectable viral load. The control group contained 20 age- and sex-matched apparently healthy volunteers. Serum innate immunity factors were assessed using Bio-Plex Pro Human Cytokine Screening Panel, 48-Plex (Bio-Rad Laboratories, Inc., USA). <italic>Results.</italic> The levels of IL-15, IL-5, VEGF were below the sensitivity threshold of the applied test-system across entire examined cohort, also observed for beta-NGF level in control group. Among all investigated indices in control group only a few of them showed insignificant differences with patients from main groups: IL-12p40, IL-13, IL-3, LIF, M-CSF, MCP-3. The general change pattern in most major cytokine levels consisted in significant hypercytokinemia in main group vs control group excepting for opposite difference for level of IL-2, TNFβ, IFNα2, G-CSF, IL-2Rα. Serum levels of Eotaxin, IL-17, IL-2Rα, IL-4 did not differ between control group and patients with viremia. Significant differences between subjects from main groups were recorded in the levels of IL-2Rα, IL-8, IP-10, MCP-1, MIG. The study confirmed the persistence of a prominent imbalance of innate immunity factors in patients with HIV infection, both in remission and in case of viral load. The chronic inflammatory landscape formed during HIV infection may be an important pathogenetic link for developing complications and syndromes associated with HIV infection.</p></abstract><trans-abstract xml:lang="ru"><p>ВИЧ-инфекция, несмотря на успехи антиретровирусной терапии, остается актуальной проблемой для изучения. Как сама инфекция или ее последствия, так и терапия, применяемая для ее контроля, влияют на иммунный ответ, изменяя его и повышая, таким образом, вероятность развития сопутствующих инфекционных процессов. Попытки персонализировать терапию ВИЧ-инфекции за счет выявления индивидуальных иммунологических и генетических маркеров строятся на анализе взаимосвязей различных функциональных профилей медиаторов иммунной системы с геновариантами ВИЧ. Ремиссии ВИЧ-инфекции во многих ситуациях удается достичь за счет выбора рациональной схемы антиретровирусной терапии, в ряде случаев — после генотипирования и определения резистентности вируса к различным ее группам. У ряда пациентов, несмотря на достижение контроля и недетектируемую вирусную нагрузку, не происходит ожидаемого восстановления иммунологического надзора и адекватного взаимодействия факторов врожденной иммунной системы. Цель настоящего исследования заключалась в сравнительной оценке состояния факторов врожденного иммунитета у пациентов с ВИЧ-инфекцией. <italic>Материалы и методы.</italic> Из 56 людей с ВИЧ-инфекцией, было сформировано 2 основных группы — с недетектируемой вирусной нагрузкой и с определяемой. В качестве группы контроля выступили 20 практически здоровых добровольцев, сопоставимых по возрасту и полу. Определение показателей факторов врожденного иммунитета в сыворотке крови проводилось с использованием набора Bio-Plex Pro Human Cytokine Screening Panel, 48-Plex (Bio-Rad Laboratories, Inc., США). <italic>Результаты.</italic> Выявленные значения IL-15, IL-5, VEGF были ниже порога чувствительности применяемой тест-системы у всего обследованного контингента, а у представителей контрольной группы такая же картина наблюдалась и в отношении beta-NGF. Среди всех исследованных показателей у людей контрольной группы лишь несколько не имели достоверных различий с пациентами основных групп: IL-12p40, IL-13, IL-3, LIF, M-CSF, MCP-3. Общая картина различий по основным исследуемым цитокинам заключалась в достоверной гиперцитокинемии в основных группах в сравнении с контрольной по большинству исследованных параметров, за исключением обратной картины по уровню IL-2, TNFβ, IFNα2, G-CSF, IL-2Rα. Сывороточные значения Eotaxin, IL-17, IL-2Rα, IL-4 не различались между группой контроля и пациентами с виремией. Между представителями основных групп достоверные различия были зафиксированы по уровням IL-2Rα, IL-8, IP-10, MCP-1, MIG. Проведенное исследование подтвердило сохранение выраженного дисбаланса факторов врожденного иммунитета у пациентов с ВИЧ-инфекцией, как в стадии ремиссии, так и при наличии вирусной нагрузки.
