In clinical practice, the early identification of the causative microorganism and the implementation of proper antibiotic treatment, as well as a comprehensive assessment of the patient’s status and management, are crucial for minimizing a risk of related severe complications and mortality. The BM etiology is characterized by a highly variable nature, contingent upon a multitude of factors such as geographic location, temporal context, patient age, immunological status, and other variables. Over the past two to three decades, advancements in conjugate vaccines and the use implementation of antibiotic treatment have brought about alterations in the spectrum of pathogens responsible for meningitis. The objective of this study was to delve into the etiological profile and prognostic implications of cerebrospinal fluid (CSF) parameters among patients with diagnosed meningitis admitted to the Neuroinfectious Diseases Division of the Infectious Clinical Hospital No. 2 (Moscow) between 2022 and 2023. A retrospective analysis of medical records of meningitis patients admitted to the Infectious Clinical Hospital No. 2 during 2022–2023 was conducted. The data were analyzed using the k-means clustering algorithm implemented in the Loginom analytics software to create a scenario, as well as OLAP cubes. The study involved the analysis of medical records for 110 patients aged 18–89 years old. Among the BM pathogens identified, <italic>Neisseria meningitidis</italic> emerged as the most prevalent in this patient cohort, accounting for 68.2% of cases. Within the group of meningococcal meningitis cases, serovariants A and W135 were the most frequently detected, accounting for 49.3% and 33.3%, respectively. Significant alterations in cerebrospinal fluid parameters, characterized by elevated cell counts and lowered glucose levels, were observed among patients with purulent meningitis resulting from serogroup A <italic>Neisseria meningitidis </italic>infections. A combination of elevated baseline cell level along with high concentrations of proteins, and lactate, coupled with the presence of D-dimer in cerebrospinal fluid, has been associated with a severe disease progression and the emergence of critical and life-threatening sequelae.</p></abstract><trans-abstract xml:lang="ru"><p>На сегодняшний день бактериальные менингиты (БМ) остаются одной из жизнеугрожающих форм инфекционной патологии и характеризуются развитием летального исхода в 4–35% случаев заболевания. В настоящее время неблагоприятный клинический исход при бактериальных менингитах зачастую связан с развитием ряда специфических и неспецифических осложнений, таких как инфекционно-токсический шок (ИТШ), отек-набухание головного мозга, вентрикулит, гидроцефалия, острое нарушение мозгового кровообращения, синус-тромбоз, ДВС-синдром, РДС-синдром, синдром неадекватной выработки вазопрессина (SIADH-синдром), синдром системного воспалительного ответа, генерализации бактериальной инфекции и др. В клинической практике минимизации рисков возникновения жизнеугрожающих осложнений и летального исхода способствуют своевременная верификация возбудителя и назначение корректной антибактериальной терапии, а также адекватная оценка тяжести состояния и маршрутизация пациента. При этом этиологическая структура бактериальных менингитов характеризуется крайне высокой изменчивостью в зависимости от географического региона, временного периода, возраста пациентов, иммунного статуса и т. д. За последние 25–30 лет на изменение спектра возбудителей менингитов также оказало влияние использование конъюгированных вакцин и широкое применение антибактериальной терапии. Целью работы была оценка этиологической структуры и изучение прогностического значения показателей спинномозговой жидкости у пациентов с бактериальными менингитами, госпитализированных в отделение нейроинфекций ГБУЗ Инфекционная клиническая больница № 2 ДЗМ за 2022–2023 гг. Выполнен ретроспективный анализ материалов историй болезней пациентов с бактериальными менингитами, госпитализированных в Инфекционную клиническую больницу № 2 за 2022–2023 гг. Анализ данных осуществлялся посредством проектирования сценария с кластеризацией по алгоритму k-means, построением OLAP-кубов в аналитической программе Loginom. В ходе исследования проанализированы материалы историй болезни 110 пациентов в возрасте от 18 до 89 лет. При анализе спектра возбудителей бактериальных менингитов в исследуемой когорте пациентов преобладала <italic>Neisseria meningitidis</italic> (68,2%). Среди пациентов с менингококковым менингитом наиболее часто верифицированы сероварианты А (49,3%) и W 135 (33,3%). Наиболее выраженные изменения в ликворе с высокими показателями цитоза в сочетании с низким уровнем глюкозы наблюдались у пациентов с гнойным менингитом, вызванным <italic>N. meningitidis</italic> серогруппы А.
