Different genetic variants of <italic>S. pyogenes </italic>can have a different set of pathogenicity factors able to change during pathogen evolution and determine virulence level for specific isolate. With a short incubation period, the disease can proceed with developing invasive infection and toxic shock syndrome with unfavorable outcome within 7 days from disease onset. The purpose of this article is to increase the doctors’ alertness to early recognition and diagnosis, which directly affects adequate treatment in a timely manner and disease outcome. The data on streptococcal morbidity in Russia and worldwide, review of laboratory diagnostic methods and pathogen genetic typing are presented. The maximum number of cases of streptococcal septicemia in Russia was registered in 2022, which accounted for 69% of all cases during the 2014–2022 observation period. The article also describes two clinical cases of fulminant invasive group A streptococcal infection in children with symptoms of acute respiratory viral infections at the onset of the disease. The results of various laboratory diagnostics methods verifying the diagnosis are presented. The genetic characterization of microbial isolates was performed by deep DNA sequencing. In the biological material from patients (including autopsy in one case), <italic>S. pyogenes</italic> sequence type ST-28, serotypes <italic>emm-1.25</italic> and <italic>emm-1.0</italic> were identified. The increasing importance of invasive streptococcal infection for health care in Russia and other countries may be associated with a possible change in dominating <italic>S. pyogenes</italic> genetic variants. In this regard, the study on circulating <italic>S. pyogenes</italic> genotypes on an ongoing basis as part of surveillance of streptococcal infection and development of vaccine for specific prevention are required.</p></abstract><trans-abstract xml:lang="ru"><p>Стрептококковые инфекции группы А доминируют среди инвазивных стрептококковых инфекций. Основной их возбудитель, <italic>Streptococcus pyogenes</italic>, достаточно устойчив во внешней среде и имеет большое число факторов патогенности, кодируемых хромосомой или передающихся путем горизонтального переноса посредством бактериофагов. Разные генетические варианты <italic>S. pyogenes</italic> могут иметь различный набор факторов патогенности, который может меняться в процессе эволюции возбудителя и определяет вирулентность конкретного изолята. При коротком инкубационном периоде заболевание может протекать с развитием инвазивной инфекции и синдрома токсического шока с неблагоприятным исходом в течение 7 дней от начала заболевания. Целью этой статьи является повышение настороженности врачей в отношении стрептококковой инфекции: своевременного распознавания и проведения ранней лабораторной диагностики, от чего напрямую зависят своевременное начало адекватного лечения и исход заболевания. В статье представлены данные анализа заболеваемости стрептококковой инфекцией в России и в мире, обзор методов лабораторной диагностики и генетического типирования возбудителя. Максимальное число случаев стрептококковой септицемии в России было зарегистрировано в 2022 г., что составило 69% всех случаев за время наблюдения с 2014 по 2022 г. Также в статье приводится описание двух клинических случаев молниеносного течения инвазивной стрептококковой инфекции группы A у детей, имевших симптомы ОРВИ в дебюте заболевания. Представлены результаты различных методов лабораторной диагностики, подтверждающих диагноз. Выполнена генетическая характеристика изолятов микроорганизмов методом глубокого секвенирования ДНК. В биологическом материале пациентов (включая аутопсийный в одном случае) идентифицирован <italic>S. pyogenes </italic>сиквенс-тип ST-28, серотипы <italic>emm-1.25</italic> и <italic>emm-1.0</italic>. Возрастающая значимость инвазивной стрептококковой инфекции для здравоохранения России и других стран может быть связана с возможной сменой доминирующих генетических вариантов <italic>S. pyogenes</italic>.
