Blood sampling was performed before surgery (preoperative period), as well as on day 7, 14 and 21 of the postoperative period. All patients with WPP were divided into two groups depending on the disease outcome during postoperative period: patients with favorable outcome (n = 34), patients with unfavorable outcome (n = 18). 68 healthy subjects were included into control group. Immunophenotyping of blood B-lymphocytes was assessed by flow cytometry and direct immunofluorescence of whole peripheral blood stained with monoclonal antibodies. It was found that patients with WPP in preoperative period contained lowered B1-cell subset compared to control group that was paralleled with low absolute B-lymphocyte count. Moreover, a higher number of activated (based on upregulated CD23 expression) peripheral blood B1-lymphocytes was observed in the patients with unfavorable than in patients with favorable WPP outcome. In addition, dynamics of changes in frequency of B-lymphocytes during postoperative period (day 7—21) varied profoundly depending on the disease outcome. In particular, patients with favorable WPP outcome during postoperative period were found to contain decreased percentage of the most B-cell subsets (including activated cells) examined, whereas patients with unfavorable disease outcome had virtually unaltered B-cell composition in the postoperative period. Moreover, count of total B-cells, naive B-cells and B2-cells negative or positive for CD23 expression was higher in patients with unfavorable outcome than in patients with favorable WPP outcome throughout entire postoperative period. Percentage of some other B-cell subsets in patients with unfavorable than with favorable outcome was also higher only at certain stages of postoperative treatment. It is assumed that such features in peripheral blood B-cell subset composition were closely linked to the disease outcome so that patients with unfavorable WPP outcome turned out to have disturbed B-cell maturation and migration in developing immune response that might be due to lowered total sensitivity of host body to postoperative antibiotic therapy.</p></abstract><trans-abstract xml:lang="ru"><p>Цель исследования — изучение фенотипического состава В-лимфоцитов периферической крови у больных распространенным гнойным перитонитом (РГП) в динамике послеоперационного лечения в зависимости от исхода заболевания. Обследовано 52 пациента с острыми хирургическими заболеваниями и травмами органов брюшной полости, осложнившимися РГП. Забор крови производили перед операцией (в дооперационный период), а также на 7-е, 14-е и 21-е сутки послеоперационного периода. В зависимости от исхода заболевания в послеоперационном периоде все больные РГП были разделены на две группы: пациенты с благоприятным (n = 34) и с неблагоприятным (n = 18) исходом заболевания. В качестве контроля обследовано 68 здоровых людей. Исследование фенотипа В-лимфоцитов крови проводили методом проточной цитометрии с использованием прямой иммунофлуоресценции цельной периферической крови с моноклональными антителами. Установлено, что в дооперационном периоде у больных РГП на фоне низкого абсолютного уровня В-лимфоцитов в крови выявляется понижение содержания В1-клеток по сравнению с контрольными значениями. При этом у больных с неблагоприятным исходом РГП наблюдается более высокое количество активированных (по CD23) В1-лимфоцитов в крови, чем у пациентов с благоприятным исходом заболевания. В наблюдаемом периоде после операции (7— 21 сутки) динамика изменений содержания В-лимфоцитов значительно различается в зависимости от исхода заболевания. У больных с благоприятным исходом РГП в течение послеоперационного периода наблюдается снижение количества большинства субпопуляций В-лимфоцитов (в том числе и активированных клеток), тогда как уровни содержания различных фракций В-клеток у пациентов с неблагоприятным исходом заболевания в послеоперационном периоде практически не меняются. В течение всего послеоперационного периода количество общих В-лимфоцитов, наивных В-клеток и В2-лимфоцитов, неэкспрессирующих и экспрессирующих рецептор CD23, у больных с неблагоприятным исходом РГП выше, чем у лиц с благоприятным исходом заболевания. Содержание некоторых других фракций В-лимфоцитов у больных с неблагоприятным исходом также выше, чем при благоприятном исходе РГП, но только на отдельных стадиях послеоперационного лечения. Предполагается, что установленные особенности в уровнях В-лимфоцитов в крови в зависимости от исхода определяются тем, что у больных с неблагоприятным исходом РГП нарушены процессы дифференцировки и миграции В-клеток в рамках развития иммунного ответа, и это может быть тесно связано с общим пониженным уровнем чувствительности всего организма пациента к послеоперационной антибактериальной терапии.</p></trans-abstract><kwd-group xml:lang="en"><kwd>widespread purulent peritonitis</kwd><kwd>В-cell</kwd><kwd>CD23 expression</kwd><kwd>postoperative period</kwd><kwd>outcome of the disease</kwd><kwd>immunophenotyping</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>распространенный гнойный перитонит</kwd><kwd>В-лимфоциты</kwd><kwd>CD23</kwd><kwd>послеоперационный период</kwd><kwd>исход заболевания</kwd><kwd>иммунофенотипирование</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
