Abstract
Influenza and other respiratory viral infections in children are a leading cause of infectious disease, characterized by high prevalence and variable clinical course. Despite the proven role of oxidative stress and cytokine imbalance in the pathogenesis of influenza infection, comprehensive age-specific studies of these mechanisms in children have not yet been conducted. The aim of this study was to evaluate the parameters of the lipid peroxidation-antioxidant protection system and cytokine profile indicators in infants, young children and preschool children with influenza.
We examined 110 children aged 1 month to 6 years diagnosed with influenza who were hospitalized at the Irkutsk Regional Infectious Diseases Clinical Hospital from December to January 2018–2019. The control group consisted of 86 healthy children. All children were tested for the following cytokines in their blood serum: IL-1β, IL-4, IL-6, IL-8, TNF-α, IFN-α, IFN-γ, and highly sensitive CRP. To assess the state of the "LPO - AOP" system, spectrophotometric and fluorometric methods of analysis were used; the content of primary, secondary and end products of lipid peroxidation (double bounds, CD, KD, TBA-active products, SB), ТАА, SOD activity, GSH, GSSG, α-tocopherol, retinol were analyzed. Children of all age groups with influenza, compared to the controls, showed a multiple increase in the levels of pro- and anti-inflammatory cytokines, as well as activation of lipid peroxidation reactions against the background of decreased antioxidant activity. Age-related features of the course of influenza in children were characterized by: activation of lipid peroxidation processes at the initial stages (maximum growth of diene conjugates, ketodienes, and conjugated trienes) and an increased level of IL-6 and TNF-α associated with broncho-obstructive syndrome in children of the first year of life; Depletion of antioxidant defense with a relative deficiency of IFN-γ in children aged 1–3 years; maximum values of the TBA-active products, IL-1β, and IL-8 in children aged 3–6 years. The data obtained substantiate the need for a differentiated approach to pathogenetic therapy for influenza in children, taking into account the child's age.
